The national registry is nice to have. I let mine go many years ago and it could be a problem if I move to a new state now where it seems that it could be required.... As far as I know it really should not affect you though unless you are near another state that requires it. For example Wisconsin requires you to have national registry but minnesota does not. If I do move to WI then I will have to test all over again to get a WI state cert.
I let mine drop because I said I would not be moving to another state anytime soon... Things change.
And national does not matter for out of state transports or transfers. You are acting under your current states/services med direction and protocols.
If you live close to another state or think you might move to one and want to work as a medic there then I suggest checking their requirements now. It could be a pain later but it is still doable.
